A+ Aging Advantage provides Elder care management in
the St. Louis area to:

  • Conduct care-planning assessments to identify problems
    and recommend solutions
  • Assess, recommend and manage in-home or facility
    resources
  • Manage the medical maze to help caregivers/children of
    elders to receive the best and most effective care for their
    patients/parents
  • Work as an advocate for the senior or disabled person
  • Act as a liaison for families to alert them of potential issues
  • Arrange and monitor in-home services
  • Facilitate communication between physicians and clients
  • Accompany and/or transport clients to medical appointments
  • Research and link clients with appropriate community
    resources
  • Coordinate respite services and support programs to avoid
    caregiver burnout
  • Monitor and inform designated caregivers of changes in
    condition
  • Assist with in-home modifications
  • Educate caregivers
  • Develop activities and systems that improve physical
    coordination and provide greater independence
  • Coordinate home modifications to help the senior or disabled
    stay in their home successfully
  • Evaluate legal and financial needs to make appropriate
    referrals
